Merge branch 'jk/rebase-am-fork-point'

"git rebase --fork-point" did not filter out patch-identical
commits correctly.

* jk/rebase-am-fork-point:
  rebase: omit patch-identical commits with --fork-point
  rebase--am: use --cherry-pick instead of --ignore-if-in-upstream
